Policy Infrastructure for AI Governance Leaders

Your board approved the AI investment. Your agents are running. Now legal or compliance asks the question that doesn't have a clean answer yet: "Can you demonstrate what your agents were authorized to do — not what they did, what they were authorized to do — under which version of your policy, effective when, at what limit?"

For most organizations today, the honest answer is: "It's in the system prompt, and we can share the current file."

That answer is becoming insufficient.

Your agents are making decisions right now. Refund approvals. Exception grants. Access decisions. Each one drawing on whatever was in the system prompt at deployment time.

There's no version history on that prompt. No record of what rule was active when the agent made a specific decision six months ago. No way to confirm that every agent updated correctly when policy changed. No audit trail that distinguishes what the agent did from what it was authorized to do.

That distinction matters. Regulators are starting to understand the difference.

The accountability gap is structural, not procedural

This isn't a process problem. You can't fix it with a better governance document or a new sign-off workflow. It's an infrastructure problem.

According to Grant Thornton's April 2026 AI Impact Survey, 78% of business executives cannot pass an independent AI governance audit within 90 days. Not because they lack policies — most have them. Because they cannot prove their agents operated within those policies. The survey identified this as the "AI Proof Gap": the distance between "we have AI policies" and "we can demonstrate what our agents were authorized to do, for every decision, at any point in time."

System prompt architectures don't satisfy those requirements. A system prompt is not a versioned policy record. It has no authorization scope. It carries no timestamp linking it to a specific decision. When that prompt changes, there's no record of which decisions ran under which version.

This is what a defensible audit trail actually requires: a record tied to every decision, not just a file that existed somewhere.
